Shared Decision Making

Well – good news and not so good news on the shared decision making front.    As many of you know, Group Health is leading a collaborative effort in the state to systematically introduce video decision aids for preference sensitive conditions (I blogged about this topic some time ago here).  The videos are being distributed consistently in many specialties, both through ordering in the EMR and live streaming on MyGroupHealth.   We have distributed over 1000 decision aids so far – pretty impressive.

But distribution is the easy part.  It is changing our clinical patterns that is tougher.   A targeted chart review suggests that much of the time there is no documentation of use of the video to help inform the decisions that patients make – half the time surgery is scheduled on the same day the video is ordered (to be delivered to the patient’s home several days later).

So on to next steps – engaging the specialty group more intensively about how to use the videos in support of the decision making process.
